    WAIVER AND CONSENT

    PLEASE READ THIS DOCUMENT CAREFULLY.

    BY AGREEING TO THIS DOCUMENT, YOU ARE PROVIDING YOUR WAIVER AND CONSENT FOR YOURSELF AND ANY ACCOMPANYING MINORS (IF ANY) AS SET OUT BELOW.

    CAPACITY AND AUTHORITY

    I, the individual listed below, have attained the minimum age of 18 years and am accepting and agreeing to this Waiver and Consent on my own behalf and/or on behalf of the minors listed below, as their parent or legal guardian, or as a duly authorized agent on their behalf.

    For any minors listed below, I represent that I have full authority to accept the terms of this Waiver and Consent on their behalf.

    ASSUMPTION AND ACKNOWLEDGEMENT OF ALL RISKS

    Lava Leap & Laser Games provides recreational services and facilities, including but not limited to lava leaping zones, laser tag arenas, obstacle courses, slides, climbing walls, and other sporting and recreational activities at its location in Singapore (collectively, “Facilities”). Additional information about the Facilities is available on Lava Leap & Laser Games’ website.

    I acknowledge and understand that participating in the activities at Lava Leap & Laser Games involves inherent risks and hazards that cannot be eliminated without impacting the essential qualities of the experience. Risks may include, but are not limited to, cuts and bruises, falls, broken bones, injuries to various body parts, concussions, spinal injuries, heat exhaustion, heart attack, other serious injuries, permanent disability, or death.

    I agree to participate in the activities and use the Facilities at my own risk, fully assuming and accepting all associated risks.

    PROPER USE OF SERVICES AND FACILITIES

    I understand and agree to use the Facilities responsibly, in compliance with all signage, directions, and instructions from Lava Leap & Laser Games staff, ensuring my safety, the safety of minors in my care, and the safety of others (“Proper Use”).

    PROPERTY AND BELONGINGS

    I take full responsibility for my belongings while at Lava Leap & Laser Games and release Lava Leap & Laser Games from liability for any loss or damage to my property during participation in any activities.

    DECLARATION OF GOOD HEALTH

    I certify that I and the minors listed are in good health and physical condition, free from any medical conditions that may affect or preclude safe participation in any activities. I understand that pregnant individuals, customers with certain pre-existing health conditions, individuals over 120 kg, or those wearing casts are not permitted to use the Facilities. I agree to inform Lava Leap & Laser Games staff if I have any doubts about our health status.

    I authorize Lava Leap & Laser Games to take all reasonably necessary steps for my welfare and that of the minors in my company in the event of a personal injury, including emergency medical treatment and transportation.

    DUTY TO CARE AND SUPERVISE

    I understand that all participants, including minors, must be in good health and condition to use the Facilities safely. I agree that minors must remain under my supervision at all times. I acknowledge that Lava Leap & Laser Games does not assume responsibility for unaccompanied minors, who remain the responsibility of their parent or legal guardian. Minors age 8 and below must be supervised by an adult 18 years or older at all times.

    INDEMNITY

    To the fullest extent permitted by law, I agree to indemnify and hold harmless Lava Leap & Laser Games and its officers, directors, employees, and agents (collectively, “Indemnified Party”) against any and all claims, actions, judgments, losses, damages, or expenses, including attorney’s fees, arising out of:

    • Any breach or non-fulfillment of my or any minor’s obligations under these terms;

    • Any negligent or reckless act or omission by me or minors in my care;

    • Any bodily injury, death, or property damage caused by our actions; and

    • Any failure to comply with applicable laws, regulations, or codes during use of the Facilities.

    WAIVER, RELEASE, AND DISCHARGE

    To the fullest extent permitted by law, I hereby waive, release, and discharge Lava Leap & Laser Games and its representatives from any claims, demands, causes of action, or liabilities arising out of or related to my use or misuse of the Facilities, except where such claims are proven to be directly due to the negligence of Lava Leap & Laser Games.

    AGREEMENT AND ACKNOWLEDGEMENT

    I agree to use the Facilities responsibly and accept liability for any injuries or damages arising from irresponsible or reckless behavior. If responsible for a minor, I agree to be bound by this Waiver and Consent on their behalf and supervise them at all times.

    AUTHORIZATION TO USE IMAGES AND PERSONAL INFORMATION

    I acknowledge that Lava Leap & Laser Games may operate security cameras, and I consent to the capture of images and videos of me and/or minors in my company. I consent to the use of such images for security purposes and agree that they may be shared with authorities if necessary.

    GENERAL

    This Waiver and Consent is binding for any future visits to Lava Leap & Laser Games and remains binding on my heirs, representatives, and estates. I agree to comply with all instructions from Lava Leap & Laser Games and acknowledge their right to deny access without refund for non-compliance or unsafe behavior.

    If any provision is deemed illegal or unenforceable, it will be amended as necessary to closely reflect the original intent without affecting other provisions.

    I, on behalf of myself and any minors listed below, acknowledge that I have carefully read, understood, and voluntarily agree to this Waiver and Consent, recognizing that I am waiving substantial rights.
